If you have an upcoming appointment at Broadway Family & Cosmetic Dentistry, we invite you to fill out your electronic new patient forms beforehand. We will then have you sign the completed forms when you arrive at your scheduled appointment.

We accept cash, checks, and credit cards. We can also help you arrange financing through the CareCredit no interest payment program.

CareCredit provides low monthly payments for dental treatment with no upfront costs, annual fees, or prepayment penalties.

Do we accept your insurance plan?

As a service to our patients, we will file all insurance claims to your carrier with supportive documentation to get them paid faster. Our office is happy to contact your insurance carrier and try to get a detailed breakdown of your policy. This allows us to present you with an accurate treatment plan estimate while maximizing your benefits as best we can.

We will do everything we can to help you receive the maximum allowable benefits under your plan. Estimates are not guaranteed, but we will always be upfront and honest about potential concerns so you can plan for possible expenses. At this time, we do not accept Medicaid.

When should children visit the dentist?

What is periodontal disease? How can I prevent it?

Periodontal disease is a bacterial infection of the gums and bones that support the teeth. Left untreated, it can lead to tooth loss. Early detection is essential, so at your next dentist visit, we’ll check for signs and treat problems before they escalate. Between visits, keep your mouth healthy by brushing and flossing regularly.

Are you a candidate for dental implants?

Patients who are in good overall health are the best candidates for dental implants. Patients will need adequate jaw bone and gum tissue to support the implants.

Patients who have diabetes, heart disease, or other health problems may not be suitable for dental implants. However, only a dentist can help you determine whether or not dental implants are right for you.
